Waiver Terms
I, the undersigned, hereby acknowledge that I am voluntarily submitting to a tattoo procedure performed by Alex Gettings of Elevated Inkness. I understand that a tattoo is a permanent alteration to my skin and body.
I confirm that I am at least 18 years of age and will present valid government-issued photo identification before the procedure begins. I am not under the influence of alcohol, drugs, or any substance that could impair my judgment or affect the tattoo process.
I understand that while Elevated Inkness follows strict sterile single-use protocols, all tattooing carries inherent risks including but not limited to: infection, allergic reaction, scarring, keloid formation, MRI complications, and dissatisfaction with the final result. I accept these risks.
I acknowledge that tattoo results vary based on skin type, aftercare compliance, sun exposure, aging, and individual healing characteristics. I have received or will receive aftercare instructions and agree to follow them.
I release Elevated Inkness, Alex Gettings, and all associated staff from any and all liability, claims, and damages arising from or related to the tattoo procedure and its outcome, except in cases of gross negligence or willful misconduct.
